We will visit with you and learn what services you are interested in then work with our community providers to connect you to those services while we continue to provide and meet all your non-medical in home care needs.

If you end up needing Hospitalization or skilled rehabilitation services, we will continue to communicate with you and your team either via phone or periodic scheduled visits to ensure that whatever services are needed upon your return home will be followed through seamlessly.
